Subcutaneous injections of testosterone esters have been the subject of interest among TRT practitioners for some time. Patient compliance has often been an issue given the difficulty in drawing higher viscosity testosterone esters into 28 – 31 gauge syringes. The movement toward SQ testosterone injections reflects the hesitation of some patients to self-administer IM testosterone and the desire of practitioners to create more stable testosterone serum levels and subsequently more stable serum levels of DHT and E2.
Short acting esters like Testosterone propionate provide greater ease of titration with daily or every other day injection schedules and patient resistance to every other day SQ injections is generally presumed lower than that of bi-weekly IM injections, while maintaining a more balanced serum level.
